Camper Health Form PDF
Must be completed by a parent/guardian for all campers and returned to the address on the Cover Letter and Confirmation Sheet. The deadline is different for each camp; see each camp's page to clarify the due date.

Medication to Camp Form PDF | Word
Must be completed by a parent/guardian for campers who are bringing any medication to camp (over-the-counter or perscription).  Mail this form to the address listed on the Cover Letter

Specialized Health Care Form  PDF | Word
Must be completed by a parent/guardian and physician for campers bringing an EpiPen and/or inhaler to camp. Mail this form to the address listed on the Cover Letter
