Sleep Away Camp Forms

After you register for camp, you will receive the forms below in the mail within 3 weeks.

  Information Packet  (Important information for parents)
Includes a packing list and directions to camp.

  Camper Health Form Due April 1, 2013
Must be completed by a parent/guardian for all campers and returned to the address below by April 1, 2013.

  Bringing Medication to Camp Form
Must be completed by a parent/guardian for campers who are bringing any medication to camp (over-the-counter or perscription).  Bring this form with you when your drop your camper off at camp or at the bus stop.

  Specialized Health Care Form
Must be completed by a parent/guardian and physician for campers bringing an EpiPen and/or inhaler to camp.  Please return this form to the address below by April 1, 2013.

  Health Form Information
If your camper health form is not received by April 1, your sleep away camp registration will be canceled.

  Meet My Camper Form
Must be completed by a parent/guardian and camper for all campers.

  Packing List
The packing list is also included in the Information Packet.
